Intake and Output Monitoring
1. Feeding administration intravenous fluids and blood transfusions will comprise intake amount.
2. Feed babies as ordered.
3. Intravenous fluids must be administered as ordered.
4. Compute on a per shift basis. However, 24-hour computation will still be performed by the night shift.
5. Add blood transfusions and PNSS flushing on the intake amount.
6. For the outputs, subtract 20cc if it comes from a diaper.
7. Vomitus must be measured accordingly.
8. Oral and gastric drainage must be measured with a syringe. Add this to the output.
9. Compute the output and check if it balance with the intake.
10. Record appropriately.
11. Refer accordingly.
12. Observe proper documentation.
